§ Pathology & physiology

Not just a problem with pain signals.

Diabetic peripheral neuropathy is a disease of both the nerve and the biological environment that supports it. Over time, metabolic derangement contributes to:

  1. Chronic hyperglycemia and metabolic abnormalities01
  2. Oxidative stress and reactive oxygen species02
  3. Endothelial dysfunction and microvascular injury (vasa nervorum)03
  4. Reduced nerve perfusion and oxygen delivery04
  5. Inflammation, mitochondrial dysfunction, and AGEs05
  6. Progressive axonal injury06

Peripheral nerves are supplied by the vasa nervorum. As the microvascular environment deteriorates, perfusion and oxygen delivery fall, and a nerve that requires blood flow, oxygen, nutrients, and energy loses the conditions it needs to function and repair.

Neuropathic-pain agents (gabapentinoids, SNRIs, and others) are valuable for symptom control, and glycemic, blood-pressure, and lipid management remain essential. But once significant neuropathy is established, options for restoring nerve function are limited. That is the gap Axon Revive™ explores.

§ The protocol rationale

Four biological targets.

  1. 1

    Microvascular

Stimulate the local tissue environment to support vascular remodeling and nerve perfusion.

  2. 2

    Oxidative stress

Address the patient's metabolic and oxidative environment as part of the protocol.

  3. 3

    Acoustic wave

Mechanical energy to tissue, investigated for effects on circulation, cell signaling, angiogenic pathways, and repair.

  4. 4

    Autologous PRP

High-concentration PRP placed perineurally under ultrasound, delivering platelet-derived growth factors and signaling proteins.

Evidence status. Axon Revive grew out of ~3 years of clinical experience. Early outcomes in our patients have been encouraging, but they represent clinical experience, not a randomized, placebo-controlled trial. It is not a proven cure, and individual outcomes vary.
